These 4 forms are required to register and swim with the Waves, Please print them, and fill them out completely.  They are in PDF File format.  If you can't print them off, please ask one of the coaches, send us an email, give us a call and we will get them to you.  Please print clearly so we enter all of your information accurately.
LESI TRANSFER FORM - needed only if you are transferring from any other USA Swim club.
